For ease of booking, we ask that you fill out ALL sections of this form, this will make sure we have enough information to accurately quote you price and time and offer you a date that is best suited to you.

If you still can't decide, just write "anyone" and we can decide which artist will be best suited to the design.
Please bear in mind that Sam books 3 months at a time and only accepts enquires when she opens her diary periodically. If you would like to be tattooed by Sam make sure you are following her and Silver Needles on Instagram for posts about when her books open.
We aim to get back to everyone on the same day but please allow for shop opening times and busy periods.
